1: FEES & PAYMENT - GST
- If a Methodist Parish registers its members and Lay Representatives through the Parish, then no GST will be charged.
- Individuals and Co-operating Ventures will be charged GST.
- All registration costs will be invoiced and direct debited.
- Please note we are unable to accept credit card payments.
2: PERSONAL DETAILS
- I am a: This category defines your qualification to be at Conference.
- In the role of: This may be the same as in the 'I am a' box , unless you have another specific role in the Conference.
- Representing: Please put name of Parish, Committee or Board that you are representing.
- Parish/Division Number: If you do not know the Parish or Division number, please leave blank.
3: CONTACT DETAILS
- Address: Please enter the postal address that a (hard copy) Conference agenda and any other information can be posted to.
- Email: Please enter the email address that your registration confirmation and any further information regarding Conference, e.g. PDF agenda, is to be sent to (if you do not have a personal email address then use your Parish email address)
4: DIETARY & SPECIAL REQUIREMENTS
- Please specify any dietary requirements. NOTE: it may not be possible for the caterer to accommodate every request.
- If you have any other special needs (wheelchair access, hearing, sight etc) please specify and indicate if you will need assistance in the section provided.
5: OPTIONAL CONFERENCE EVENTS
- There are a number of optional events which occur during Conference. Tickets must be purchased in order to attend any of these events.
- Payment (where applicable) is made at the time of registration.
- Event tickets will be included with your Conference name tag when collected on arrival at Conference.
6: OPTIONAL AGENDA FORMAT
- Tick the boxes to order either a paper copy or a PDF copy of the agenda, or both.
- You must choose one of these options in order to receive an agenda.
- (Note: ordering an agenda is optional)
